Saw this machine at auction today.  I had to reduce the resolution quite a bit to post the images here.  The label says "General Sales, LTD.  Ireland Island, Bahamas.  US pat # 3,285,380 Belgium patent # 658,897.  The patent image is only vaguely similar to the actual machine although the reel mechanism does look similar to a Mills mechanical system with the three index wheels on the right side.

I was able to get the reel mech to work by tripping a lever on the right but saw no sign of power on any of the relays. (not sure if the outlet was hot or not).  The coin slot looks to be about the size of a 50 cent piece even though there is the tape showing "nickels only".  There is also a button on the front of the machine by the lock but it's not obvious what's that for?

The upper part of the exterior case is somewhat reminiscent of the Jennings electromechanicals from the late 1970s and early 1980s, but after seeing the electronic junkyard inside, I have no idea.
