MEI Bill Jam

My MEI bill validators will jam sometimes leaving the end of the bill torn and laying on top of the cash can. The bill is still intact but the end is torn in about 5 places about 1/2" long and it looks like something is smashing the bill into the ridges on top of the cash can. This has happened on several different machines/cash cans, so it's not specific to any certain one. What could be causing this?
