I have an IGT S+ that is set up for quarters, but when i put a nickel in, it registers as a valid coin. Also, I am not sure if this is related, but every once and a while when I put a quarter in, it is not registered as a credit, but goes into the hopper anyway.
I have tinkered with the sensitivity on the CC, but it doesn't make a difference.
Any ideas?

What is the sample coin in the CC?
Post a picture of it as well please.

When you put a quarter in and it's not counted or credited and ends up in the hopper, this means the coin comparator accepted the coin but the optical sensor didn't detect it. Looks like you have two separate problems. Check to be sure you have the 0.25-cent coin guide in the optical sensor. If you have a larger guide this can cause miscounts. Might try cleaning the optics or as a last resort replace the optics. As for the coin comparator, I'm not much help there. You might just replace it.

The sensitivity control must always be turned fully counterclockwise.

Is there a way to determine what guide I have? Does every denomination have its own guide?

There should be a number on it. Guide for quarters is P/N 58405402
