Title: S+ Player Tracking Plate Information

Have a Double Diamond Deluxe S+ that I'm trying to complete and am missing the player tracking cover plate.  Not having any luck finding a part number for this in any docs I have available.  What should I be looking for?  Appears to be about 19.5" wide.

Depends on your machine, several types were made. pictures are worth a 1000 words.

I have many types available, just want to be certain of which one is correct.
