That game is BB1 "Wild Zone" I think?
That would use OS SSG-000-1440
I could be wrong...

Those 3 reel steppers use OS E1.11 if I recall.
Looks like your Game card says Wild Zone E1.1

If you can, post a clear photo of the game card.

Yesterday I had saw a Topic from 2010 or 2012 on this same game...I wish I saved it. :duh:

There was these old D1.3, E1.1, E1.3, and G1.3 operating systems.
A little later on, they started using E1.11 when the Tsetse games came out.
I'd be trying everythng I have.
I had an old BB1 Zeus game that gave me problems once...same thing...couldn't get it to run.
My headaches were gone when I threw it in the garbage can!  :pos_computer:   :rotfl:

This game...you had it working before, or no?

I think when I had this issue I had to change the batteries . Probably reseated the CPU as well.

The Game card is not reporting the OS requirement, see open error 6?  Game card is probably bad.  Have you made an image and check the header on a hex editor to see if the image looks right?
