The first 5 are the same numbers but the last 3 are 500, 600, and 700. I assume 500 is reel 1, 600 is 2 and 700 is 3

Your chip is for 2 coins. Do you have 2 coin glass?

Yes. Running the base paytable reel 1 and 3 are in stop position 01 and reel 2 in stop
Position 02. That should pay 4 and it pays nothing

Have you checked your strips to make sure the are installed correct? Notch on the strip to the locating notch on the reel..

Wrong strips! Those are probably for a multi line game. Correct strips are pictured below.
