What the hell is this? Sigmatron LV 1004

I took this out of something, well a slot machine... I neatly laid it on the stand of another slot. It's been there for a year. Now I can't remember where the heck it came from.  :hissyfit:
Anyone know? It looks like a part from a progressive unit but I'm not sure.

It may have come out of my Bally EVO if that helps. I just hate to store it away and then really not know what it is when I "find" it again.  :rotfl:

Thanks!!! Now I can label it, and hopefully put it somewhere. I still don't remember what it came out of... maybe one of my three Game Makers I had, or from the now long gone EVO.
