Hi and welcome to Newlifegames.
This site is primarily for "newer" (non full mechanical) slot machine, BUT there are a few of us experienced in that area.
I moved your post to "Other Reel Games" and I'll inform our resident PRO about your problem.
My first guess is your bottom coin slide is jammed or gummed up. Sometimes people put oil or grease on them, which is a no on.
Can you CAREFULLY pull out the mechanism, and take a couple photos: a good close up of the front of the mechanism, and one of the front bottom right as well? There may be a thumb screw holding it in, so check the lower area. Pull the mech straight out and be careful with the reel strips.

There is another web site that's intended for all mechanics machines "coinopcollectorforum.com" However it's VERY slow with response and you many not hear anything for awhile! If you decide to join that, PM me for the code they will ask.
But let's start here first! We may be able to help you out faster.
Consider becoming a regular contributing member, which helps pay costs to keep this site up and running so you can keep your machine up and running :)

Cam524,

As shorttrackskater mentioned.  It could be the slides are gummed up or jammed.  If the machine was laid down a coin can turn sideways in the sides.  It could be a number of other things too.

When was the last time this machine worked properly?

Has the machine sat for a long period of time?

When you play the game can you hear "5th." click?  If not, the slides may not be re-setting.

You can go to the coinop forum and search the 5th. click and that will give you some direction to start.

I checked and yeah it unwinds and does 3 clicks slows to a fourth but no 5th click it feels like whoever worked on it for him greased it and stuff and it hasn't worked for like 5 years we all play it constantly but it either wouldnt pay out or it would but the wrong ammount

Cam524,

If grease was used that is a No-No on these mechanical machines.  Grease tends to get sticky and harden over time.  You may need to disassemble, clean and re-lube.  Both 3-1 one oil and Vaseline work.  Payout slides need to clean and free of lubricant.
